  • Authorization form to utilize for client consent to disclose PHI 

    • You may only share a client’s information with individuals or entities who are specifically authorized. If no HIPAA exception applies, you must obtain a signed authorization. For example, you cannot share a client’s care details with an assisted living facility or family member unless they are explicitly authorized to receive that information.

    • This form needs to be used for any individuals who are authorized after the care consult is completed. If a family member is named during the care consult, no separate form is required.
